Coronary artery bypass grafting (CABG) is a form of surgery that helps in providing the blood flow to the heart. It is performed on individuals who experience severe coronary heart disease (CHD), also called coronary artery disease.

How to prepare for the treatment?

  • Certain tests will be performed for your coronary artery bypass graftings (CABG) such as blood tests, echocardiography, a chest x-ray, an EKG (electrocardiogram), cardiac catheterization, and coronary angiography.
  • The doctor will advise you on what you can eat or drink before the surgery.

Procedure involved
Generally, the following procedures are followed while conducting CABG:

  • You will have to change into a hospital gown and empty your bladder before the operation.
  • Your heart and blood pressure status will be checked, and a healthcare professional will start an intravenous (IV) line in your arm, and anaesthesia will be given.
  • A breathing tube will be inserted through your mouth into your lungs to connect you to a ventilator. The machine will help you in breathing during the surgery.
  • For an open-heart surgery, the chest area will be shaved, and the surgeon will make an incision down the centre of the chest to just above the navel.

Coronary artery bypass graft surgery--on-pump procedure

  • For CABG, the doctor will have to stop the heart. A heart-lung bypass machine will be utilized to pump the blood while your heart is stopped.
  • Through a small opening made in the aorta, the surgeon will conduct the bypass graft procedure by sewing one end of a vein section. The other end over a slight opening carried out in the coronary artery just below the blockage.
  • You may have to undergo more than one bypass graft depending on the number and severity of blockages.
  • As the surgery gets completed, the surgeon will shock your heart with small paddles to restart your heartbeat. The blood will re-enter your heart and tubes to the heart-lung bypass machine will be removed.

Coronary artery bypass surgery--off-pump procedure

  • The surgeon will stabilize the area around the artery to bypass the heart with a unique instrument.
  • The heart will continue to function and pump blood during the surgery.
  • Through a small opening made in the aorta, the surgeon will conduct the bypass graft procedure by sewing one end of a vein section. The other end over a slight opening carried out in the coronary artery just below the blockage.
  • You may have to undergo more than one bypass graft depending on the severity of blockages.
  • As the heart beats again, the doctor will watch how efficiently the graft is functioning.

Both the Common Procedures

  • The doctor will drain the blood and other fluids from around the heart using tubes.
  • The skin over the sternum will be sewed back together, and incisions will be closed.

What to expect after the treatment?

  • You will be taken to the recovery room and later to the ICU and monitored for several days.
  • You will be connected with machines that will continually reveal your breathing rate, blood pressure, electrocardiogram (ECG) tracing, other pressure readings, and oxygen level.
  • You will likely have a tube in your throat that's linked to a ventilator for a day or two to help you breathe until you can breathe on your own.
  • You may receive pain medicine if you are hurting.
  • You may be on IV (intravenous) medicines to maintain your blood pressure and heart functions and look out for any bleeding problems.
  • Once your doctor has separated the breathing and stomach tubes and you are stable, you may start to consume liquids. You can start eating solid foods as soon as you can stand for them.
  • When your healthcare provider concludes that you are ready, you will be transferred from the ICU to a surgical unit or acute care unit.
  • Your recovery will continue there. Your activity will be gently increased as you get out of bed and walk around for lengthier periods.
